We know that your time is important, and we make every effort to stay on schedule so we respectfully ask patients to be prompt and ask that you give at least 24 hours notice if you must cancel or reschedule an appointment. Initial Visit And New Patient Forms

Are you new to Guthrie Family Dentistry? We are pleased to welcome you to our Guthrie, OK practice and look forward to meeting you! Below are a few things that will help you understand what to expect, and feel free to call us with any questions you may have before your visit:

Please arrive fifteen minutes prior to your first appointment to complete new patient paperwork if you have not done this prior to your appointment.
Patients with specific health conditions should bring a list of all medications they are currently taking.
Children under the age of eighteen years old must be accompanied by a parent or guardian.
